How to refer

We accept referrals from all qualified health professionals. To refer, please select the following link to the appropriate referral form. We will only accept referrals for care home residents that have followed our care home pack guidance. For our FLIP guidance see here.

We aim to see urgent referrals within two weeks of receiving referral and routine referrals within six weeks of receiving the referral.
