Children’s Community Nursing Service

The Children’s Community Nursing Team provides a range of services to support children, young people and their families at home and in the community. We aim to prevent or reduce hospital admissions. Your child has been discharged from the Paediatric Assessment Unit, Paediatric Day Surgery unit or Ward 5 today. This means that our doctors were happy that your child is well enough to be cared for at home.

Being at home is the best place for your child to recover however we understand that you may have questions or concerns following discharge and may require further support at home.

Follow up Support
Follow up support may involve advice over the phone, home visit or occasionally you may be asked to bring your child back to the hospital for a review. If you need advice or have any questions regarding your child’s visit to hospital please call the Community Nurses on 01908 996521. There is an answer phone which is responded to frequently throughout the day. The service operates:
Mon–Fri 8:30am-4:30pm and Sat 9am-12pm.

Support – Out of Hours
If you require advice or support out of hours contact the Paediatric Assessment Unit on 01908 996368. Please do not return to the unit without phoning first as this is unsafe for your child. If you are ever seriously concerned about your child’s condition the Emergency Department (A&E) is the safest place for their initial assessment. Inform the staff that your child has recently been seen
by the Paediatric team.

After 24 hours any further concerns should be dealt with by contacting your GP or urgent care services.
