Referral & Inclusion Criteria

Frailty SDEC is designed for patients who are:

• Over 65 years of age
• CFS (Clinical Frailty Score) > 5
• Presenting with frailty syndromes (e.g., falls, reduced mobility, delirium, polypharmacy)
• Medically stable (NEWS < 5), fit to sit and able to mobilise with one person’s help
• Likely to be discharged within 6 hours
• In need of an urgent holistic MDT review
• Able to attend the unit via their own transport

Exclusion criteria include suspected stroke, fracture, infectious conditions (e.g. COVID-19, Norovirus), challenging behaviour requiring 1:1 care, or the need for time-critical head scans.
